A time is Scheduled with the clients and Colbrow’s Case Manager to Conduct a Care Assessment
An OH&S Assessment is also Conducted
In consultation with the client a support plan is developed focussing on a person centred approach
The Business Operations Manager oversees the implementation of the program
Care Co-ordinator and care staff are assigned to the program
Rosters are established and program
Co-ordinated from Colbrow's head office
Regular feedback is sought/provide from/to the client on a regular basis
